She defended the master's thesis on a subject: "Death mechanisms at an electric trauma". In 1935 Esfir Semenovna together with the husband went to Kuibyshev where she worked as the assistant at department of physiology of the Kuibyshev medical institute. In 1937 her husband was arrested willows 1938 shot extrajudicially, in 1956 it was rehabilitated.

In 1938 Esfir Semenovna as the wife of "the enemy of the people" was arrested, the apartment was taken away, subsequently it was given a semi-basement in which she lived 15 years. After release from prison she could not get a job within 2 years.

The practical skills and theoretical preparation received in department of professor P.K. Anokhina were useful at the front. As the neurosurgeon Esfir Semenovn worked on 3 fronts: Southwest, Volkhov, Second Ukrainian up to August, 1945. Began the neurosurgical activity under the leadership of the Kiev professor Pavel Semyonovich Babitsky at first as the assistant, and then and independently.

At the 52nd army the separate company of medical strengthening (SCMS) which had to render surgical to the help at head wounds was created. Neurosurgeons, maxillofacial surgeons, ophthalmologists entered this company. Earlier at such wounds sent to the back, wasted precious time. Specialized offices were developed in 10-12 km from a front line. Exactly here as the neurosurgeon E.S. Tolmasskaya was also appointed. Here she received the first award - the medal "For Services in Battle".

Its doctoral dissertation included the data obtained at inspection of patients with cherepnomozgovy injuries and the neurophysiological researches conducted by it earlier. In 1948, work was ended, and Esfir Semenovna gave it to the head of the department professor P.K. Anokhin. He gave Esfir to Semenovna appreciation as to the research associate and highly appreciated results of its scientific work.

After protection of the doctoral dissertation it continued to work at institute of neurosurgery. It developed an original method of registration

electric activity of a cerebellum of the person, helping to diagnose cerebellum tumors. E.S. Tolmasskaya was among erudite pioneers of use of methods of an elektroentsefalografiche-sky research in clinic.

Soon, however, she again, as well as many others at this time, underwent discrimination, was fired from institute of neurosurgery, was left without the favourite business both without means of support, and only after I.V. Introduced electrophysiologic methods new for those times for diagnosis of mental diseases, prepared 2 doctors and 15 candidates of science. It published, except many scientific articles and collections, 3 monographs, including "About nervous mechanisms of coordination of somatic and visceral functions of an organism".
